For your first appointment, please remember to bring the following:
Valid referral from your GP or Specialist
Medicare card and/or pension card
Current Updated list of medications, copies of recent blood test results, blood glucose record and glucometer, scans including the images
For Ministry of Transport Paperwork to be completed, please bring your blood glucose diary with 5-7 days of documented glucose readings, a recent eye report and blood tests no older than 3 months.

Monday: Inner West Specialist Centre, Burwood (02) 9715 6404 (8:30am-5:00pm)
Wednesday: RPA Hospital Diabetes Centre (02) 9515 5888 (9:00am-4:30pm)
Tuesday (fortnightly) and Thursday: Sydney Medical Specialist Group - Bankstown (02) 9790 0272 (8:30am-5:00pm)
Friday: Bankstown Hospital Diabetes Centre (02) 9722 8360 (9:00am-4:30pm)
Consultation Fees
Schedule of fees will be advised upon booking your appointment.
You will receive a medicare rebate to cover approximately half of the consultation fee.
Concessions are available for aged pensioners (over 75-80 years)
Cancellations
Cancelling or rescheduling an appointment must be done at least 24 hours prior to the appointment
